A new public COVID-19 vaccine clinic will open from noon to 8 pm in the Lewis County Mall, 151 NE Hampe Way, Chehalis on Wednesday, April 28. Washington residents age 18 or older can get their first or second Moderna vaccine shot at no cost in the former Sears store location.
Pre-registration is required. Use this link to register. The Washington State COVID-19 Assistance Hotline is available for those unable to register online. Call 1-800-525-0127, then press # for English or 7 for Spanish. Hotline hours are 6 a.m. to 10 p.m. Mondays, and 6 a.m. to 6 p.m. Tuesday through Sunday, and on holidays.
The clinic is being staffed by the Washington Army National Guard. Current planning calls for conducting two public walk-in clinics per week at this location, depending on vaccine availability and public demand. Future clinic notices will be posted @LCPHSS on Twitter and Facebook, at Lewis County’s COVID-19 information page, and at the Department of Health’s vaccine locator page.
This clinic was developed in cooperation with the Washington State Department of Health, the Washington Army National Guard, Lewis County Division of Emergency Management, and Lewis County Public Health & Social Services.
