Providence Swedish Chehalis Family Medicine will be offering free sports physicals to Lewis County middle- and high-school students who need the required physical exam to participate in school-sanctioned athletics during the upcoming academic year. The no-cost exams will be offered for the third consecutive year at Providence Swedish Chehalis Family Medicine (931 S. Market Blvd., Chehalis) on Wednesday, August 19, from 4:30-6:30 p.m.
To participate, students must provide completed sports physical forms with parental signatures. Forms can be obtained at the clinic or by contacting Providence Swedish Chehalis Family in advance of the event (360.767.6300). Students under 16 years old must have a parent on site during the exam. Students who are 16 years old or older can attend without a parent but must have a parental signature on their forms. These sports physicals do not replace recommended ‘well-child’ exams conducted by a family medicine provider.
